BOOKING & CANCELLATION POLICY

The fee for the Initial Assessment is $375. At the end of the session, we will review options for packages and if you’d like to do any of those, I’ll apply the cost of the Initial Assessment to the package.

I ask for partial payment ($75) at the time of scheduling, to hold the space on the calendar. I will process the second part of the payment at the time of your session.  This partial payment is non-refundable.

INSTRUCTIONS

#1: Confidential Health History: Please be as thorough as possible.

#2: Food journal – please fill this out for 3-5 consecutive days. Be as thorough as possible, and fill in as much information as you can in the third column titled “Mood/Digestive changes”. This is the beginning of your work with me, to understand how your body is responding to the food you’re eating. If you feel good and then eat a meal and feel bad either emotionally, digestively, energy-wise, anything, write it down. And vice versa – if you feel bad and then feel good after a meal, write it down. Don’t try to switch anything up from the way you’ve been eating. Eat as you do typically. That will give me the best picture.

#3: A disclaimer and agreement for you to sign – this outlines my policies and other important information. Please read it fully and sign it – and don’t hesitate to let me know if you have any questions.
